Role Overview: The Weekend Hospice Liaison serves as a vital connection between Grace Healthcare Services and patients, families, and referral sources during weekend hours. This role is responsible for facilitating timely hospice referrals, conducting patient and family consultations, and ensuring a smooth transition to hospice care.
Key Responsibilities:
- Coordinate with the admissions team to ensure timely evaluations and admissions.
- Respond promptly to weekend hospice referrals from hospitals, skilled nursing facilities, and physician offices.
- Meet with patients and families to explain hospice services, answer questions, and provide emotional support.
- Gain access to patient H&P, and documentation, to aid in providing information to potential eligibility and communicate all findings to the interdisciplinary team (clearly providing as much information to Grace clinical team).
- Maintain strong relationships with weekend hospital staff, discharge planners, and RN case managers.
- Meet and greet weekend staff in SNF, and hospitals as designated by leadership team; coordinate with Community Liaisons who covers designated accounts.
- Collaborating with senior leadership to develop and implement effective sales strategies, as well as execute plans that are created by Community Liaison
- This is a support position to enhance Grace's presence during weekend hours; this candidate will work collaboratively with a team of Community Liaisons.
- Maintain working knowledge of industry trends, product knowledge, sales techniques, changes in hospice regulations, best practices and competitor activities.
- Attend training, conferences, industry events to network, enhance organization visibility.
- Represent Grace Health in a professional and compassionate manner.
- Ensure all sales, business development and marketing activities are compliant with regulations and ethical guidelines.
- Accurately document all referral and patient interactions in the CRM.
- Communicate clearly and effectively with on-call staff, ensuring the needs of our patients are being met.
- Maintain EPIC, and all intake systems.
- Maintain HIPAA, STARK, and other health care rules and regulations.
- Meet/exceed all KPI and performance expectations for this role as set by the VP of Business Development Marketing and Director of Sales.
- Maintain a focus on a people-first mentality and promote that culture throughout the training and development programs.
- Adhere to the practice of confidentiality with regard to patients, staff, customers and all aspects of the organization.
- Ensure that Grace Healthcare Services best practices are incorporated into all job activities/ responsibilities.
- Perform other duties as assigned.
QUALIFICATIONS:
- Relevant experience required.
- Minimum 5 years' experience in hospice/healthcare, with a solid background in business development, training, designing educational curriculum, and strong leadership/management skills required.
- Strong understanding of hospice/healthcare industry including hospice philosophy, operational guidelines & best practices, as well as BD compliance requirements.
- Excellent communication, presentation, and interpersonal skills.
- Strong analytical and problem-solving skills.
- Ability to commute within the state of NJ, and other states as applicable.
